Community Support Services

We provide dedicated, worker-led Community Support Services through our signature Soulcial Advocacy initiative. As a fully credentialed, PAVE-approved, and NPI Type 2 registered 501(c)(3) organization, we deploy certified Community Health Workers (CHWs) to deliver high-touch social care management directly inside our communities. We specialize in providing comprehensive, non-clinical social navigation across diverse populations of focus.

CHW Care Coordination

Multi-format 1-on-1 social coaching delivered face-to-face in the community and via telephone.

Housing Stabilization Advocacy

Specialized housing navigation, move-in security deposit deployment, and ongoing tenancy retention support to actively prevent evictions.

Medication Safety Screening

Evidence-based, in-home medication reconciliation to identify dangerous drug interactions and safeguard overall health stability.

Care Transitions

Provide rapid-response support for adults safely returning home immediately following a hospital stay or clinical discharge.
